Environmental Health Practitioner Portfolio registration form

Use this form to enrol on CIEH's Environmental Health Practitioner Portfolio.

You should only complete this form if you meet one of these criteria:

• You have successfully completed a CIEH-accredited BSc in Environmental Health
• You have successfully completed a CIEH-accredited MSc in Environmental Health
• You have completed the taught elements of one of the above
• You are in the final year of a BSc EHP Apprenticeship
• You are in the 'year out' of an accredited BSc (sandwich course)

Please note: to gain Registered EHP status, you will need to have passed the practical food identification exam (usually part of an accredited degree course).

You will need to provide evidence of your qualification or enrolment on your course with this registration form.
